Each service provided under the HCS Program must be delivered in accordance with the approved Individual Plan of Care (IPC), Individual Service Plan (ISP) and the HCS Program service definitions set forth in the HCS program provider agreement.

SUPPORTED EMPLOYMENT

RESPITE

RESIDENTIAL ASSISTANCE
The following are the three types of residential assistance that can be provided:
Supported Home Living – provided to individuals residing in their own residence or the residence of their natural or adoptive family; or to individuals receiving foster care services from the Texas Department of Protective and Regulatory Services.
HCS Foster/Companion Care – provided in a private residence by a foster or companion care provider who lives in the residence and who is not providing these services pursuant to a contract with Texas Department of Protective and Regulatory Services.
Residential Support
– provided to individuals by daily shift staff or by an individual staff member with periodic relief staff. (3-bed and 4-bed models)
